Why patients come here
Most people arrive after years of cetirizine, levocetirizine, montelukast and a nasal spray used when it gets bad. None of those medicines is wrong. They are simply doing a different job from the one the patient thinks they are doing.
An antihistamine blocks the message. It does not change the immune system that keeps sending it. Stop the tablet and the symptoms return, because nothing underneath has changed.

The work here is in two parts, in this order. First, establish what you are actually allergic to — by history, by skin prick testing, and where needed by specific IgE blood testing. A great many people are treated for years for allergies they do not have. Second, decide whether immunotherapy is appropriate. It is not right for everyone, and you will be told so plainly when it isn't.

What we treat
- Allergic rhinitis — year-round or seasonal sneezing, blocked nose, post-nasal drip, and the morning congestion antihistamines only partly touch
- Allergic asthma — wheeze, night cough and breathlessness triggered by dust, pollen or pets, assessed with spirometry rather than guesswork
- Chronic urticaria and angioedema — hives lasting more than six weeks, where the aim is to find a trigger and stop the steroid cycle
- Atopic eczema — itchy, recurring skin disease, often alongside asthma or rhinitis in the same patient. Eczema, hives and contact reactions are what most people mean by skin allergy
- Food allergy — confirmed testing and a written action plan, including what to do about a reaction and not only what to avoid
- Drug allergy — documented evaluation of suspected reactions to antibiotics, painkillers and anaesthetic agents, so the label on your file is accurate
- COPD, bronchiectasis and chronic cough — including post-tuberculosis lung disease and occupational or pollution-related disease
- Pneumonia, bronchitis and interstitial lung disease — acute chest infection through to the scarring disorders, with the follow-up that confirms a lung has actually recovered
- Lung cancer screening, pulmonary hypertension and embolism — screening for long-term smokers, and breathlessness that turns out to be the circulation rather than the airways
- Sleep-disordered breathing — snoring, witnessed apnoeas and daytime sleepiness

Testing panels written for European patients over-represent grasses that barely feature here and under-represent what is genuinely in this air. In and around Hyderabad the recurring culprits are house dust mite, which peaks in the humid and cool months and explains the classic hour of sneezing after waking; moulds, especially in the fortnight after the monsoon retreats; Parthenium hysterophorus — congress grass — on every unbuilt plot and roadside, which causes airway symptoms and a contact dermatitis on the face and forearms; and cockroach allergen, badly under-recognised and a significant driver of childhood asthma in dense housing.
Then there is what is not an allergen at all but makes every allergy worse: construction and road dust, incense and mosquito coils indoors, biomass cooking smoke, and the city's winter air.
